The programme is designed
to help NHS organisations implement an integrated
approach to medicines management that will support
people with long-term conditions across whole
health communities.
The IMMP is co-ordinated
by an experienced team based at the National Prescribing
Centre (NPC) in Liverpool. This team has over
four years experience of working collaboratively
on national medicines management projects with
NHS practitioners, independent contractors, managers
and patients.
We already know about many
improvements which, if implemented, can have a
direct impact on the following priority areas:
- Reducing hospital admissions
and equally importantly re-admissions
- Reducing length of hospital
stay
- Supporting self-care
and care planning
- Disease management, in
particular, cardiovascular disease and diabetes
- Improving choice and
access to a range of medicines management services
- Managing risk and patient
safety
- Increasing service efficiency
in primary care and hospitals thereby saving
both time and money
With the close involvement
of practitioners, the IMMP aims to accelerate
the implementation of local improvements for long-term
conditions. This will be achieved by enabling
service commissioners, providers and users to
think differently about how medicines management
services are delivered.
Management of long-term conditions
is now and will remain a priority for the NHS
and patients. Medicines management is not an optional
extra that can be bolted on but needs to be considered
as an integral part of service planning and delivery.
The IMMP approach provides:
- A common purpose that
helps unify activities, processes and communication
to deliver day-to-day frontline improvements
in medicines management across whole health
communities
- Advice, guidance and technical
support available from a named member of the
team at the National Prescribing Centre
- The opportunity to learn
from and build-on existing knowledge in a way
that enhances innovation and creativity
- Support for organisations
and individuals in an open learning environment
that is sensitive to local priorities but delivers
results
Teams involved in some of
our existing work programmes, such as Guildford
and Waverley PCT, West Gloucestershire PCT and
Nottingham City Hospital NHS Trust, have gone
on to attain national
recognition for their pioneering work in medicines
management
