Medicines Reconciliation
The NPC has worked with NHS trusts for a number of years to improve communication of medicines information across healthcare interfaces. In February 2008, following the publication of a patient safety solution for medicines reconciliation on adult admission to hospital by the NPSA and NICE, the NPC launched its own guide to the implementation of medicines reconciliation. The guide will help you to understand the importance of obtaining accurate and timely information about patients' medicines, and the part that each of us has to play in ensuring that every patient receives a personalised service as far as their medicines are concerned. Medication Review
Prescribed medicines are the main intervention for the prevention and treatment of ill-health provided by the NHS. It is important that medicines are prescribed appropriately for patients and that they continue to get the most from medicines after they are prescribed. Personalising Medicines Management Medicines management is about enabling patients to make the best possible use of medicines, it needs to be in place for each patient. The principles of good medicines management should apply in all settings where patients access and use medicines, and it should travel along patient pathways across all care interfaces.The NPC recently published a guide about the design and delivery of medicines management services. It is written for commissioners and providers as a tool to support improvement and personilisation of services. |
